    It was down to a lot of things. From memory there were several causes, including a very coarse thread for the gudgeon pin nut, the slot for the nut retaining cotter being horizontal instead of vertical, the split in the cotter not being deep enough so was bent at the stress raiser point, and that the nut was backed off to align the slots in the nut and gudgeon pin, rather than fit a different thickness spacing washer.

    I don’t think that anyone would want to admit to owning Tangmere. She’s a scruffy, pig ugly, smelly, smokey, clanking beast...but I love her to bits - which she is often in, or flinging them around the Network.
    The ultimate drama queen, never a dull moment.

    Damned good acceleration, that! Is it just the dampness of Shap cutting, or is there a flange lubricator in the vicinity. Most videos I've seen show locomotives slipping at the same point just before the summit...

    Likewise, she was a real smoke machine, one of the first engines I rode behind on the mainline (I believe the third)
