Well a continuous stream of water running out of an injector drain when there is no obvious reason why water should be going anywhere does suggest - at a guess - that an injector feed water valve has failed but that is quite unusual. That said, if there is no means to maintain the right level of water in the boiler it's little wonder that the fire had to go or you would also lose a plug or two.

 

I think in this instance the feed from the tender has been inadvertantly left on so not an injector failure

Whilst I appreciate we are interested to know what caused the failure , until there is word from the owners we may need to be patient and not engage in too much speculation

 

Leaving the feed from the tender on 'inadvertently' is a strange thing to do, is it not unless for some reason you want to keep the injector cool? But anyway, a sad incident.
