    Oh! I'd have thought it would have been a no brainier for some sort of RAIB report. As a guard and carriage shunter, I'd have been interested to read any findings just as learning points.

    Don't RAIB investigate only if someone has been hurt or could have been hurt?
    We don't know how a coach became detached, but the braking system brought both parts of the train to a stop, as such systems are intended to do. The stop would have been sudden, but not as sudden as in a collision.

    The TL,DR is that just because something is reportable doesn’t inevitably mean it generates an RAIB investigation.

    I have no knowledge of the present circumstances, and make no comment on them.

    Their approach appears to be to focus on incidents that have more general learning points or where the cause is unclear. Based upon recent history, I can equally well imagine RAIB taking no action, doing a safety digest, or a full investigation depending on what they encounter - and with no knowledge of what happened on the day, or why, I also make no comment o what outcome is likely.

    Does anyone have any strictly factual information, such as where the incident occurred, and which set of carriages was involved? Just hoping it wasn't the teak train!

    I believe it was at Moorgates. I also believe the reason is known but I don't know what it was. Back in the day, divided trains weren't that unusual; that's why guards vans carried an emergency screw coupling. When it happened, make sure all is OK, re-couple the train, and carry on.

    Such an incident poses a significant risk of serious injury or death to anyone passing through the corridor connection, so the RAIB may take an interest on that basis.

    It was set of Mark One vehicles, from a photo of the incident I’ve seen showing one carriage detached a short distance behind the rest of the train.

    Back when I was directly involved in safety management, it was a very specific requirement from RAIB to report unplanned uncoupling of corridor fitted vehicles.

    All of which requires reporting to RAIB. RAIB do not investigate every report, and much will depend on the information accompanying the report as to whether RAIB perceive value in undertaking such a report. There are other, and often far more timely, ways of sharing information between operators.

    Is it possible to describe, factually, how it is possible for this to happen (in a hypothetical scenario) without speculating on what did happen in this situation? Do the NYMR use buckeyes between carriages?

    There is a requirement to report divided trains to both RAIB and ORR. Whether these are followed up depends, probably, on the detail and findings of the railway's own internal investigation.
