    Although the track defect led to the derailment, the locomotive rostered for passenger duty that day was a recently rebuilt freight engine. It had once been an 0-6-0, but the trailing wheels had been removed and the remaining axles placed 6" further to the rear in order to allow it to traverse tight curves in coal sidings. In this state, it was intended to be driven at speeds "not greater than 15 or 16 miles an hour". The engine had worked six times between Cockermouth and Worthington on the day of the accident. "She" was noted by the crew to "sway terribly". Indeed.
